Create a Survey

You can plan events, make a survey or poll, give students a quiz, or collect other information in an easy, streamlined way with Google Forms. You can create a form from Google Drive or from an existing spreadsheet that can record the responses to your form.
Create a form from Drive
To poll, survey, quiz, or otherwise collect information using a form in Google Drive:
1. Go to Drive at drive.google.com.2. In the top left, click New, hover over More, and choose Google Forms. Don't see New? 
You may be using an older version of Drive. Try clicking Create in the top left, then Form.3. A new form will automatically open.4. In the form template, you can add any questions you want. You can also organize your form by adding headers and dividing your form into several pages.
Add a question
To add a question to your form, click the arrow next to the Add item button and select from the following question types:
Text — respondents provide short answersParagraph text — respondents provide longer answers multiple choice — respondents select one option from among several checkboxes — respondents select as many options as they’d like choose from a list — respondents select one option from a dropdown menuScale — respondents rank something along a scale of numbers (e.g., from 1 to 5)Grid — respondents select a point from a two-dimensional graduate — respondents use a calendar picker to enter a dateTime — respondents select a time (either a time of day or a duration of time)
Add a theme to an existing form:
Open the form. Click Change theme in the toolbar. Scroll through the templates in the panel on the right and click the theme you’d like to add to your current form. Your new theme will automatically be applied and you can continue editing your form.
Create a custom theme if you want even more control over how your form looks, you can create your own custom theme.
Open the form.Click Change theme in the toolbar.Find one of the existing templates in the panel on the right to use as a starting point and click Customize under the name of that template.Select a section of the form in the panel on the right to begin editing that section. Some of the options you can customize include:Adding custom images to the header and the page backgroundChoosing font, font size, font color, and paragraph alignment for the form textChoosing a color for the form and page backgrounds
